DCHS Band Absence Reporting Form

This form is required for any absence to rehearsal, performance, or required band event. Absences include situations that require students to leave early or arrive late to a rehearsal, performance, or event. The decision to designate an absence excused or unexcused rests solely with the director. Parents and students will be expected to adhere to DCPS, DCHS, and Band of Pride attendance and absence notification policies.
